About the Role

The Care Coordinator works on the inbound call center team and is often the first person patients speak with. The role centers on answering incoming calls, routing more complex concerns to the right team member, and supporting communication across internal and external care teams. Care Coordinators help create a supportive, patient-first experience by listening carefully, solving problems, and connecting patients with the right resources.

This is an hourly, non-exempt position.

Location

This position is fully remote.

Responsibilities

  • Handle incoming patient calls and route escalated concerns to the right person or resource.
  • Deliver strong customer service by listening attentively, answering questions, and escalating issues when needed.
  • Reach out for, request, and gather required documentation from patients and community providers.
  • Assist care teams with non-clinical and operational tasks.
  • Receive patient referrals through inbound fax.
  • Record patient conversations and important details accurately.
  • Complete assigned follow-ups on time to keep care coordination moving smoothly.
  • Take on additional duties as needed.

Required Qualifications

  • High school diploma or GED, or a higher level of education.
  • Background in resolving escalated customer service issues.
  • Working knowledge of Google Workspace tools, including Google Docs, Gmail, Google Calendar, Google Slides, and Google Sheets.
  • Ability to use Google Sheets for creating, editing, sorting spreadsheets, and handling basic analytical formulas such as VLOOKUP.
  • Experience working with phone and fax systems.
  • Prior EMR experience; Athena or Epic experience is preferred.

Preferred Qualifications

  • Associate degree or above.
  • Typing speed of at least 40 words per minute.
  • Experience as a Medical Assistant, Certified Nursing Assistant, Pharmacy Technician, or in another clinical or administrative healthcare role.
